AI scribe comparisons & buying guides.
Honest, practitioner-focused comparisons of the AI medical scribes built for independent primary care — by price, note structure, EHR handoff, and workflow fit. We make Stream, and we'll tell you where a competitor is the better choice.
Features and pricing, side by side
Alphabetical, not ranked. We make one of these, so a league table from us would be worth exactly what you'd expect — the buying guide makes actual recommendations, with the reasons and the cases where we lose.
| Product | Price / month | Free tier | EHR handoff | What you get |
|---|---|---|---|---|
| Doximity Scribe | Free | Entirely free | None — copy and paste | One note per visit. The recording is discarded once the summary is produced. English only. |
| Freed | $39 – $119 | 7-day trial, no card | Copy and paste | One note per visit. Starter is capped at 40 notes a month. |
| Heidi | Free · around $150/month | Yes — a real one | Not on the Free or Clinician plans | Template-driven notes, customisable, with a genuinely free tier and broad language support. |
| OpenEvidence Visits | Free | Free to verified US clinicians | Copy and paste | One note per visit, with guideline references in the assessment and plan. |
| Suki | Around $299–$399+/month, not published | No | Yes — integrations available | Voice-first notes plus voice commands for driving the EHR, with real integrations. |
| Twofold | $69/month billed annually | A $0 plan giving 7 days of full access, no card | Claims integration available | Notes across many note types with CPT/ICD-10 coding included. No audio retained. |
| Stream (River Records) | $149/month | 30-day trial, no card | Copy and paste | Subjective/Objective/A&P split per problem, filed to a chart organised by problem rather than by date. |
Competitor pricing checked on 22 September 2026; each name links to our full comparison, which links to that vendor's own pricing page so you can check rather than trust us. Prices move — Heidi's went from roughly $99 to around $150 in February 2026, and Twofold's entry tier is no longer listed. Suki publishes no pricing at all, so that figure comes from third-party reports rather than from Suki. Stream does not offer EHR write-back and is not SOC 2 certified; if either is a requirement, that rules us out and we would rather say so here.
Stream vs. Freed
A direct, feature-by-feature look at how Stream and Freed compare for primary care documentation.
OpenEvidence’s scribe and Stream
OpenEvidence is clinical decision support with a scribe feature; Stream is a documentation platform. What each is for, and why many clinicians use both.
Stream vs. Heidi
Heidi has a real free tier and a polished product. Stream is a problem-based chart, not a cheaper scribe. Where each one wins, and who should pick which.
Stream vs. Suki
Suki offers EHR integrations and voice commands at roughly $299–$399/month. Stream is about half that and organized around problems rather than encounters. An honest comparison.
Stream vs. Doximity Scribe
Doximity Scribe is free to verified US clinicians. Stream is a problem-based chart with document ingestion. Where each fits, including when the free option is the right answer.
Stream vs. Twofold
Twofold is $69/month with coding bundled. Stream is priced like a chart because that is what it is. An honest comparison for independent practices.
These guides are published by River Records, the maker of Stream. We aim to represent every tool fairly, including where competitors are the better fit. Pricing and features change frequently — verify current details on each vendor's own site before deciding.
